Immunization Requirements

  • The Alabama Department of Public Health, Immunization Division has issued revisions to the immunization rules for entry into public schools (pre-K programs-12grade).

    The pneumococcal vaccine is now required for children entering pre-K programs. This includes four doses of the pneumococcal vaccine prior to enrollment.

    All students 11 and older entering the sixth grade must have a tetanus-diphtheria-acellular pertussis (Tdap) vaccine. Each student 11 or older who enters the sixth grade will be required to have a new updated certificate of immunization (Blue Slip).If a student entering 7th grade has not had a previous Tdap, they are required to have it prior to entry into school this school year.

    Documentation of Varicella (chickenpox) vaccination or immunity will be required for all students.

    These vaccinations may be obtained from your private physician, 91HUB County Health Department or Northeast Alabama Health Services Clinics.
